You must go to the Morioka Immigration Office to replace a lost, soiled, or damaged Residence Card. Please carry your passport with you during this time for identification. Please go within 14 days of the incident. If lost, please report the loss to the police as well.

The photo must show the applicant facing forward, without any hats or head coverings. The background must be plain, and the photo easy to see.

It must be taken within the last three months.

Once a card is reported lost to the police, they will submit to you a certificate.
