● Example of Necessary Documents for a Mobile Phone Application (for an individual adult who has completed Alien registration connecting with NTT DoCoMo)

1. Identification

a) Any one of the following; driver’s license, credit card (in your name), student ID card, Alien Registration Card*
*Your eligibility to remain in Japan must be at least 90 days. Your listed and current addresses must be identical.
b) Either a health insurance card and a utility bill / receipt or a personal seal registration card and a utility bill / receipt.

2. Name Seal¹ and Bank Book²

(1) The seal with which you opened the account from which payments will be made.
(2) Bankbook for the account or something showing the above account number.

3. Costs

a) Contract handling fee (3,000yen)
b) Cost of the phone unit (varies)
*You may be required to pay a deposit (no-interest, c. 100,000 yen or less on a 1 year contract)
